Two-factor authentication is now account level, not just by login method

We've updated how two-factor/OTP login works, so that DigiCert ONE can better support compliance rules around Secure Software Manager. Two-Factor login as a setting is now global to the account; affecting all login methods. For example, when two-factor authentication is enabled, both Single Sign-on and Standard sign-on will now require a One-time-passcode. For SSO users, this prompt will appear when DigiCert ONE loads. The OTP is logged for audit purposes. The interface has also been updated to use display cards, instead of a table, to better communicate statuses.

Various bugfixes and UI tweaks were deployed.
